Jayshri Gamoth, M.D.

Dr. Jayshri Gamoth is the CEO and Medical Director of A to Z Dermatology.

With over 10+ years of experience, Dr. Gamoth has become one of Arizona’s leading physician’s in the treatment of general and surgical skin conditions. Dr. Gamoth is skilled in complex surgical techniques and specializes in the treatment of various skin cancers.

Her private practices cover all areas of dermatology including cosmetic treatments such as Botox, Fillers and IPL Photo Rejuvenation. As an educator, Dr. Gamoth consistently shares the importance of a healthy skin regime with her local community. Her desire is to offer affordable healthcare to all who are in need. In her own words,

“My favorite part of practicing medicine is to help my patients find solutions and care for them on a personal level.”

Dr. Gamoth has participated in various research studies and presentations on topics ranging from skin cancer to common skin disorders. As a highly qualified medical professional, Dr. Gamoth strives to promote a comfortable atmosphere in which her patients receive the highest quality of care.

In her spare time, Dr. Gamoth enjoys spending time with her family. She also enjoys community events, music, traveling, and non-profit work.

Richard Levine, M.D.

Richard Levine M.D. is a board-certified dermatologist with over 22 years of experience. Dr. Levine practiced in Colorado for 20 years and recently moved to Arizona and joined A to Z Dermatology. He is very pleased to be part of a growing practice that offers cutting edge complete dermatologic care in a warm inviting environment.

Dr. Levine is a dual U.S. and Canadian citizen. He completed his dermatology training at McGill University in Montreal in 1997. Previous to that he trained in internal medicine at the University of Toronto. He graduated from medical school at the University of Western Ontario. He completed An undergraduate degree in biology there as well. Dr. Levine is board certified by the American Board of Dermatology. After completing his dermatology residency, he completed additional extensive Training in Mohs Skin Cancer Surgery, and cosmetic surgery and became A fellow of the American Society for Mohs Surgery and a fellow of the American Academy of cosmetic surgery. He enjoys all facets of Dermatology and sub-specializes in minimally invasive cosmetic surgery.

In his spare time, Dr. Levine enjoys traveling, outdoor activities including hiking and cycling, and spending time with family.

Joyce Kate Forest, M.D.

Dr. Joyce Kate Forest is a board-certified physician with experience in clinical adult and pediatric dermatology including surgical dermatology since 2013. Her passion for dermatology includes skin cancer prevention, early detection, and treatment. She is skilled in the evaluation, diagnosis, and treatment with a strong commitment to high standards of care. She is active in her community services including skin cancer screening.

Dr. Forest holds a degree in Bachelor of Science in Physical Therapy and Doctor of Medicine. She completed a residency in Family Medicine at Atlanta Medical Center then further pursued specialty training in Dermatology in the state of Iowa. She also has experience in Emergency Medicine and Hospital Medicine.

Dr. Forest also has interest in Aesthetic Medicine and provides anti-aging and cosmetic procedures all to help patients look and feel their best. In her spare time, she enjoys traveling, Zumba or Mixxfit cardio training, volunteers at her church and is a dog lover.

Patricia Bernal, NP

Patricia Bernal is a board certified nurse practitioner. She is a native, born and raised in southeast Arizona.

She attended Arizona State University obtaining her BSN. She then decided to further her education by pursuing a MSN and becoming a family nurse practitioner. This gave her the opportunity to do what she loved in a much broader scope.

She has a wide range of experience in dermatology, family practice and hospice. As an AZ native, she knows the affect of sun/sun damage, skin cancer, as well as a vast array of skin disorders.

Her passion is to promote prevention, prevent skin cancer by bringing community awareness, as well as offer treatment and support to patients with skin disorders to achieve optimal outcomes.

Hobbies: Enjoying her grandkids and family, walking, biking, organizing and decorating.

Francisco Trejo

Francisco, aka Frank, Trejo is originally from El Paso, Texas. He has been a medical practitioner since 1998 and is a graduate of the University of Nebraska College of Medicine, Inter-Service Physician Assistant program. During the completion of over 20 years of service in the United States Air Force, he was an enlisted F-16 aircraft maintainer. Later he completed the Physician Assistant program, practicing medicine in a wide range of settings to include family medicine, emergency/urgent care, occupational medicine, and women’s care. Being a full-time Dermatology provider since 2008, he has mentored family medicine residents, physician assistant, nurse practitioner and pharmacy students. He has traveled to the Dominican Republic, Ecuador and Guatemala as a medical humanitarian team member. He first called Arizona home in 1986 while assigned to Luke AFB. Again in 2002 while stationed at Davis-Monthan in Tucson and finally completed his military career at Luke AFB from 2005-2008. He enjoys his family time with his wife and boys. He tinkers around with his Corvairs and his beloved 1967 International pickup.

He is a member of the Arizona State Association of Physician Assistants and Society of Dermatology Physician Assistants.
